Involuntary psychiatric commitment is disturbing in a free society, presenting challenges to competing values of civil liberties, personal well-being, and public safety. In this book, psychiatrists Miller (psychiatry, Johns Hopkins Sch. of Medicine) and Hanson (psychiatry, Univ. of Maryland Sch. of Medicine) skillfully confront the topic in a sweeping journalistic account using case studies, research findings, legal history, and interviews with psychiatric gatekeepers. The work begins with a review of diverse perspectives on commitment issues as held by experts including E. Fuller Torrey, the American Psychiatric Association, and adherents of Thomas Szasz. A historical review of civil commitment laws and policies is provided with reference to abuses that predate modern legislation and civil rights safeguards. The bulk of the book then covers the realities of psychiatric commitment in various settings and innovative criminal justice diversion programs. Additional topics include an insightful discussion of the impact of mental illness and involuntary treatment on public safety concerns such as gun control, violence, and mass murder. The book concludes with informed suggestions for public debate. Impressive for its breadth, this book complements Stephen Seager's more dramatic Behind the Gates of Gomorrah.
