MacKinnon (psychiatry & behavioral sciences, Johns Hopkins; Trouble in Mind) presents an important, clinically oriented yet practical study of why antidepressant treatments sometimes fail in certain patients. The author's approach isn't abstract, as he offers a series of composite case histories that illuminate the various reasons those prescribed antidepressants alone don't receive relief: incorrect dosing, an inaccurate diagnosis, the need for a different treatment modality, and even learned helplessness. He lays out these examples richly, giving the appropriate information about the larger psychosocial context for each one. The book helpfully points out what clinicians might have missed; questions they didn't ask that affected their thinking that they were dealing with major depression, when in fact there were other issues that needed to be addressed.
